Admission Requirements:
- Diploma or GED
- Previous completion of HIM 111 and HLT 145 with a grade of “C” or better or currently enrolled
- Overall GPA 2.0 or higher (use high school if no college work attempted)
- Placement scores indicate readiness for minimum of ENF 2
- Submit an application for the Phlebotomy Program online. Deadline for application submission will be April 15th for summer semester and July 15th for fall semester. A new cohort begins at the beginning of the summer semester each year and another begins at the beginning of the fall semester each year.
- Student should submit their Hepatitis B immunization record(s) along with their program application if they desire to utilize those points earned in the application process.

Additional Admission Requirements
Additionally, the following immunization and other requirements of the clinical facilities must be COMPLETE by no later than the first MDL 106 class orientation meeting:
- Proof and record of three Hepatitis B vaccinations OR proof of immunity by titer
- Proof and record of two Hepatitis A vaccinations OR proof of immunity by titer OR proof of the first in the series being received within the past six months
- Proof and record of two MMR vaccines OR proof of immunity by titer
- Proof and record of negative TB test (two-step)
- Proof and record of seasonal flu shot
- Proof and record of current health insurance coverage
- Proof and record of two varicella vaccinations OR proof of immunity by titer
- Proof and record of DTP (diphtheria, tetanus, pertussis) vaccinations OR proof of immunity by titer
- Copy of current BLS for Healthcare Provider (CPR) certification card, back and front (certification must be current and maintained until the completion of MDL 106)
- Criminal and sexual background checks will be performed mid-term in MDL 105
- Drug testing will be performed mid-term in MDL 105
